LoTS care

Aims and objectives
The aim of the LoTS-Care trial is to evaluate the clinical and cost effectiveness of the system of care as delivered by a Stroke Care Coordinator (SCC) to patients and their carers living at home in the community on improved psychological and functional outcomes as compared to standard SCC practice.
Primary Objective:
The primary objective of the trial will be to determine whether the system of care improves psychological outcomes for patients requiring long-term care at home.
Secondary Objectives:
The secondary objectives are:
-To determine whether the provision of the system of care improves functional outcomes for patients requiring long-term support at home;
-To determine whether the provision of the system of care improves psychological and functional outcomes for carers of patients requiring long term support at home;
-To assess whether the system of care is cost-effective based on patient outcomes from both health/social care and societal perspectives.
